Ultraprotective Ventilation via ECCO2R in Three Patients Presenting an Air Leak: Is ECCO2R Effective?

نویسندگان

چکیده

Extracorporeal CO2 removal (ECCO2R) is a therapeutic approach that allows protective ventilation in acute respiratory failure by preventing hypercapnia and subsequent acidosis. The main indications for ECCO2R are COPD (chronic obstructive pulmonary disease) exacerbation, distress syndrome (ARDS) other situations of asthmatics status. However, procedure not extended to those ARDS patients presenting an air leak. Here, we report three cases leaks with were successfully treated using new device. Case 1 polytrauma patient developed pneumothorax during the hospital stay, case 2 post-surgical bronchial fistula after Ivor–Lewis esophagectomy, 3 COVID-19 who spontaneous being hospitalized prolonged time. allowed mitigating VILI (ventilation-induced lung injury) significantly improved acidemia, allowing time native heal. Although further investigation needed, our observations seem suggest can be safe effective connected mechanical ARDS-associated leaks.

ECCO2R, a french national survey

Introduction ECCO2R (extracorporeal dioxide carbon removal) is an extracorporeal decarboxylation technology described in 1978. Physiological studies showed that 50% of produced CO2 were eliminated. Potential indications are: ultra-protective mechanical ventilation (MV) for acute respiratory distress syndrome (ARDS) and hypercapnic patients at risk of failure of noninvasive ventilation (NIV). Be...

“Awake” ECCO2R superseded intubation in a near-fatal asthma attack

BACKGROUND Near-fatal asthma attacks are life threatening events that often require mechanical ventilation. Extracorporeal carbon dioxide removal (ECCO2R) is, beside extracorporeal membrane oxygenation (ECMO), a well-established rescue option whenever ventilation gets to its limits. But there seems to be very rare experience with those techniques in avoiding mechanical ventilation in severe ast...

Current Applications for the Use of Extracorporeal Carbon Dioxide Removal in Critically Ill Patients

Mechanical ventilation in patients with respiratory failure has been associated with secondary lung injury, termed ventilator-induced lung injury. Extracorporeal venovenous carbon dioxide removal (ECCO2R) appears to be a feasible means to facilitate more protective mechanical ventilation or potentially avoid mechanical ventilation in select patient groups. With this expanding role of ECCO2R, we...

An extracorporeal carbon dioxide removal (ECCO2R) device operating at hemodialysis blood flow rates

BACKGROUND Extracorporeal carbon dioxide removal (ECCO2R) systems have gained clinical appeal as supplemental therapy in the treatment of acute and chronic respiratory injuries with low tidal volume or non-invasive ventilation. We have developed an ultra-low-flow ECCO2R device (ULFED) capable of operating at blood flows comparable to renal hemodialysis (250 mL/min). Comparable operating conditi...

ACUTE HYPERCAPNIC RESPIRATORY FAILURE Pathophysiology and causes

NIV is an effective strategy in specific settings and in selected population with AHRF. To date, evidence on ECCO2R is based only on case reports and case-control trials. Although the preliminary results using ECCO2R to decrease the rate of NIV failure and to wean hypercapnic patients from invasive ventilation are remarkable; further randomized studies are needed to assess the effects of this t...
