Ultraprotective Ventilation via ECCO2R in Three Patients Presenting an Air Leak: Is ECCO2R Effective?

Extracorporeal CO2 removal (ECCO2R) is a therapeutic approach that allows protective ventilation in acute respiratory failure by preventing hypercapnia and subsequent acidosis. The main indications for ECCO2R are COPD (chronic obstructive pulmonary disease) exacerbation, distress syndrome (ARDS) other situations of asthmatics status. However, procedure not extended to those ARDS patients presenting an air leak. Here, we report three cases leaks with were successfully treated using new device. Case 1 polytrauma patient developed pneumothorax during the hospital stay, case 2 post-surgical bronchial fistula after Ivor–Lewis esophagectomy, 3 COVID-19 who spontaneous being hospitalized prolonged time. allowed mitigating VILI (ventilation-induced lung injury) significantly improved acidemia, allowing time native heal. Although further investigation needed, our observations seem suggest can be safe effective connected mechanical ARDS-associated leaks.
